Individual instructions
Your login ID will be the first letter, capitalized, of your first name and your last name, first letter capitalized - here is an example JDoe. If you have an email address on file, you should receive an email with your user name and password. If you did not receive an email, you will need to email or call the office for your user name and password, wvfuneralboard@wv.gov or 304-558-0302. You will need to furnish your license number and last four digits of your social securiy number before we will give the password out. Once logged in, please change your password. If any of your information is incorrect, please email the office with your correct information.
Facility instructions
